Our Team of Caring Professionals

Our Interdisciplinary Team (IDT) is led by medical director, Dr Gary Tubbs. The team consists of the patient’s Primary Care Physician, Registered Nurses, Certified Nursing Assistants, Chaplains, Social Workers and Volunteers. Our patients can receive care in any of the seven southwestern Idaho counties we currently serve: Ada, Boise, Canyon, Elmore, Gen, Owyhee, and Payette. As individuals are faced with the diagnosis of terminal illness, the majority would choose to live the remaining days of their life in the comfort of their own home. All too often, this does not happen simply because people are not aware of the availability of hospice and the support it provides for the patient, their loved ones and/or their caregivers. Four Levels of Care:

• Home care
• Continuous care for crisis intervention
• Inpatient care for patients in hospitals and skilled nursing facilities
• Inpatient respite care
